Fibrøs dysplasi

Baggrund – Fibrøs dysplasi er en godartet fibro- ossøs lidelse, der er karakteriseret ved en patologisk omdannelse af normalt knoglevæv til fibrøst væv indeholdende irregulære umodne knogletrabekler. Fibrøs dysplasi debuterer oftest i barndommen og udvikles i ungdomsårene for senere at stabilisere sig og blive inaktiv efter afsluttet vækst. Lidelsen præsenterer sig som en asymptomatisk langsomt voksende hævelse, som kan medføre kosmetisk skæmmende ansigtsasymmetrier, displacering af tænder og funktionelle gener. Behandlingen varierer fra henholdende behandling til kirurgisk konturering af de afficerede områder, afhængigt af udbredelse og symptomer. Patienttilfælde – En 12-årig dreng blev henvist til Kæbekirurgisk Afdeling, Aalborg Universitetshospital, med henblik på diagnostik og behandling af en asymptomatisk hævelse i overkæben. Diagnosen fibrøs dysplasi blev verificeret ved biopsi og histologisk undersøgelse. Patienten fik foretaget kirurgisk konturering af det afficerede område som følge af en kosmetisk skæmmende ansigtsasymmetri. Ved toårs postoperativ kontrol fremstod patienten uden kosmetiske eller funktionelle klager. Konklusion – Fibrøs dysplasi er en godartet fibro-ossøs lidelse, hvor kæberne er blandt de hyppigst afficerede knogler. Langvarig regelmæssig klinisk og radiologisk kontrol anbefales, idet udviklingen af fibrøs dysplasi kan resultere i kosmetiske og funktionelle gener, svækkelse af knoglerne med risiko for kæbefraktur, og i meget sjældne tilfælde kan lidelsen undergå malign transformation. Det aktuelle patienttilfælde har været til toårs klinisk og radiologisk kontrol, og der har vist sig stationære forhold.

Fibrous Dysplasia – Case study with an observation period of 2 years: Background – Fibrous dysplasia is a benign fibro-osseous lesion, characterized by a pathological replacement of normal bone tissue by an excessive amount of benign fibrous tissue concomitant with irregular immature bony trabeculae. Fibrous dysplasia usually develops during childhood and early adolescence and stabilises when skeletal maturity is reached. The lesion presents itself as a slow-growing and painless swelling of the affected area causing potential cosmetic deformity, displacement of teeth and functional problems. Depending of the nature and size of the lesion, treatment varies from regular recall examinations to surgical intervention of the affected areas. Case study – A twelve-year-old boy was referred to the Department of Oral and Maxillofacial Surgery, Aalborg University Hospital, for diagnosis and treatment of an asymptomatic swelling of the maxilla. Biopsy and histological examination revealed the diagnosis of fibrous dysplasia. As a result of cosmetic deformity the patient was treated in the affected area with surgical contouring. At 2 year post-operative follow- up, the patient appeared without cosmetic or functional complaints. Conclusion – Fibrous dysplasia is a benign fibro-osseous lesion with the jaws being among the most commonly affected sites. A long-term clinical and radiologic follow-up is recommended considering development of fibrous dysplasia can cause cosmetic and functional deformities, weakening of the bone with the risk of jaw fracture and in rare cases malignant transformation. At 2 years post-operative clinical and radiographical follow-up the current patient showed stationary conditions.
