Desmoplastisk ameloblastom

Baggrund – Desmoplastisk ameloblastom er en sjældent forekommende benign odontogen tumor ofte med et aggressivt vækstpotentiale og en høj recidivfrekvens. Tumoren optræder hyppigst hos midaldrende som en asymptomatisk hård og langsomt voksende hævelse i incisiv- og præmolarregionen. Radiologisk ses en diffust afgrænset patologisk forandring bestående af radiolucente og radiopake områder. Tumoren vokser infiltrativt langs knogletrabeklerne, hvorfor radiologisk afgrænsning af tumor ofte er vanskelig. Desmoplastisk ameloblastom behandles med kirurgisk resektion. Imidlertid er recidivfrekvensen høj, hvorfor langvarig klinisk og radiologisk kontrol er nødvendig. Patienttilfælde – En 63-årig kvinde blev henvist fra egen tandlæge til Kæbekirurgisk Afdeling, Aalborg Universitetshospital mhp. diagnostik og behandling af en asymptomatisk og langsomt voksende hård hævelse i underkæbens frontregion. Radiologisk undersøgelse viste en diffust afgrænset radiolucent og radiopak struktur strækkende sig fra regio 3- til -2. Histologisk undersøgelse af biopsi viste forandringer forenelige med desmoplastisk ameloblastom, hvorfor der efterfølgende blev foretaget resektion af tumor og de involverede tænder. Konklusion – Desmoplastisk ameloblastom er en sjældent forekommende odontogen tumor, som klinisk og radiologisk kan være vanskelig at differentiere fra andre patologiske forandringer i kæberne. Histologisk undersøgelse er nødvendig for at kunne stille den endelige diagnose og vælge den korrekte behandlingsstrategi.

Desmoplastic ameloblastoma Background – Desmoplastic ameloblastoma is a rare benign odontogenic tumour with an aggressive growth potential and a high recurrence rate. The tumour occurs mainly in middleaged patients as a painless hard and slowly growing swelling in the incisor and premolar region. Radiographically, a diffuse ill-defined mixed radiolucent and radiopaque lesion is seen. The tumour has an infiltrating growth pattern in the cancellous bone for which reason radiographical demarcation is difficult. Desmoplastic ameloblastoma is treated with surgical resection. Because of the high recurrence rate, prolonged clinical and radiological control regime is necessary. Case study – A 63-year-old female was referred from her dentist to the Department of Oral and Maxillofacial Surgery, Aalborg University Hospital, for diagnosis and treatment of a hard painless and slowly growing swelling in the anterior region of the mandible. Radiographic examination revealed a diffuse ill-defined mixed radiolucent and radiopaque lesion from the region of mandibular right canine to the left lateral incisor. Histological examination of a biopsy revealed changes compatible with desmoplastic ameloblastoma. The tumour including the associated teeth was resected. Conclusion – Desmoplastic ameloblastoma is a rarely occurring odontogenic tumour which has many comparable clinically and radiologically characteristics of other pathological jaw lesion Histological examination and diagnosis is necessary in order to choose the appropriate treatment.
