Behandling ved agenesi af mand iblens 2. præmolar

Agenesi af mandiblens 2. præmolar er hyppigt forekommende. Således mangler 4,1 % én eller begge af underkæbens 2. præmolarer. I nærværende artikel redegøres for principperne for diagnostik, behandlingsplanlægning og behandling. De forskellige typer af behandlinger beskrives: Bevarelse af den primære 2. molar længst muligt, hel eller delvis spontan eller ortodontisk lukning, implantatbehandling, behandling med konventionel eller plastretineret bro og autotransplantation af præmolar. Endvidere præsenteres en opgørelse fra Regionstandplejen i Region Nordjylland af behovet for de forskellige behandlingsmodaliteter for 403 patienter med i alt 575 agenesier af mandiblens 2. præmolar. Opgørelsen viste, at der ikke var behov for behandling svarende til de fleste agenesiområder (68 %). I 27 % af tilfældene var der indikation for implantatbehandling efter tab af den primære 2. molar og afsluttet vækst. Der var sjældent indikation for ortodontisk lukning (1 %) og behandling med konventionel eller plastretineret bro (1 %). I 3 % af tilfældene kunne der først lægges en endelig behandlingsplan senere, idet tandskiftet på undersøgelsestidspunktet ikke var afsluttet. Meget få (0,3 %) ønskede ikke behandling. Det må imidlertid antages, at et betydeligt antal patienter er færdigbehandlet i den kommunale tandpleje eller ikke er henvist til Regionstandplejen, idet der ikke forventes et fremtidigt behandlingsbehov.

Treatment of patients with congenitally missing mandibular second premolars: The mandibular second premolar is frequently congenitally missing. Actually, 4.1 % has agenesis of one or both of the mandibular second premolars. The principles of diagnosis and treatment planning are described. Also, the various treatment modalities are described: Maintenance of the deciduous second molar as long as possible, complete or partial spontaneous or orthodontic space closure, implant treatment, treatment with conventional or resin-bonded bridge, and autotransplantation of teeth. Results from a study performed at a centre in the northern part of Den- mark for patients with congenitally missing teeth are presented. The study involved 403 patients with 575 congenitally missing mandibular second premolars. It was revealed that no treatment was indicated in most cases (68 %), while implant treatment was indicated in 27 % of the referred patients. Orthodontic space closure and treatment involving conventional or resin-bonded bridge were rarely indicated (1 %). Infrequently, it was too early to perform treatment planning (3%), because the patients were in the mixed dentition. Very few patients were not interested in treatment. However, the patient group is selected, because only patients with an expected treatment need after the age of 18 years are referred to the centre.
