Malignt lymfom manifesteret som endodontiskparodontal læsion

Baggrund – Maligne lymfomer optræder i sjældne tilfælde med symptomer, der kan give anledning til forveksling med endodontiske eller parodontale lidelser. Patienttilfælde – Her præsenteres patienttilfælde, hvor maligne B-celle-lymfomer optræder i de parodontale væv. Det ene manifesteres som en hævelse i gingiva og bliver hurtigt korrekt diagnosticeret efter biopsitagning. Det andet bliver først diagnosticeret og behandlet som apikal parodontitis og derefter som marginal parodontitis, inden den korrekte diagnose bliver stillet på en biopsi, som tages i forbindelse med et parodontalkirurgisk indgreb. Konklusion – Tandlæger bør være opmærksomme på, at der kan være tale om malignitet, hvis der ikke er det forventede respons på en endodontisk eller parodontal behandling.

Background – Occasionally, malignant lymphomas may present with symptoms that could indicate endodontic or periodontal disease. Case Study – In this article, we present a cases of B-cell lymphoma in the periodontal tissues. In one case, the lymphoma presents as a gingival swelling, and the correct diagnosis is established after biopsy. In the other case, the lesion is initially diagnosed and treated as apical periodontitis and later as periodontal disease. A biopsy taken during periodontal surgery finally establishes the correct diagnosis. Conclusion – Dentists should consider the possibility of malignancy, if lesions fail to respond to endodontic or periodontal treatment.
