'Drug holiday' af antiresorptiv medicin ved tandekstraktioner hos cancerpatienter

Introduktion og formål – Knoglestyrkende (antiresorptiv) medicin ordineres bl.a. til patienter med maligne tilstande i knoglerne. Disse patienter har stor risiko for udvikling af osteonekrose i kæberne efter tandekstraktion. Hvis en patient i antiresorptiv behandling skal have foretaget en tandekstraktion, pauseres patientens antiresorptive medicin normalt inden ekstraktionen. Det er imidlertid ukendt, om en pausering (drug holiday) har indflydelse på risikoen for udvikling af osteonekrose i kæben efter tandekstraktion med primær slimhindedækning. Formålet med denne artikel er på baggrund af foreliggende litteratur at afklare betydningen af en drug holiday i relation til tandekstraktion på patienter i højdosisantiresorptiv behandling. Metode – Nøgleord og søgning blev udarbejdet i samarbejde med videnskabelig søgekoordinator på Copenhagen Trial Unit og inkluderer litteratur i relevante videnskabelige databaser. Søgning på PubMed blev tillige gennemført. Gennemgang af eksisterende videnskabelig litteratur – Der fandtes ingen forsøg, som undersøgte betydningen af drug holiday i relation til tandekstraktion for udvikling af osteonekrose hos cancerpatienter i højdosisbehandling. Drug holiday omtales i flere systematiske oversigtsartikler. Konklusion – Der findes hverken evidens for eller imod anvendelse af højdosisantiresorptiv drug holiday i relation til tandekstraktion med primær slimhindedækning. Betydningen er væsentlig at få afklaret, da antiresorptiv medicin formindsker smerter, risiko for spontanfrakturer og død i forbindelse med malign sygdom.

Antiresorptive therapies effectively reduce the incidence of skeletal fractures and bone pain in patients with metastatic bone cancer and multiple myeloma. Patients in high dose antiresorptive treatment are at high risk of developing osteonecrosis of the jaws (ONJ) after tooth extraction. Presently, the antiresorptive treatment is discontinued in relation to the tooth extraction, known as drug holiday – even though it is unknown whether a drug holiday plays a role for the development of ONJ after tooth extraction with primary mucosal cover. The purpose of this article is to clarify the importance of a high dose antiresorptive drug holiday in relation to tooth extraction for the development of ONJ. Methods – Keywords and search were made in collaboration with Copenhagen Trial Unit and included literature in all relevant scientific databases. Review of the literature – We did not find any trials or systematic reviews, testing whether antiresorptive drug holiday could reduce the development of ONJ in relation to tooth extraction in cancer patients at high dose antiresorptive treatment. Conclusion – At present there is no evidence for or against the use of a high-dose antiresorptive drug holiday in relation to tooth extraction with primary mucosal cover for the development of ONJ.
